PTA Required Forms & Documentation
Each Seminole County Council PTA/PTSA operates as a 501c3 non-profit organization under the FL PTA Group Exemption. When you need official documentation for your PTA/PTSA (501c3 non-profit organization), you will recieve those documents from FL PTA.
All Seminole County Countil PTA/PTSAs are required to submit annual compliance items to FL PTA to certify that it is operating within the requirements to maintain their 501c3 non-profit organization status under the FL PTA Group Exemption.

REquest Your PTA Operating Documents from FL PTA
Each Seminole County Countil school-based PTA/PTSA operates under the FL PTA Group Exemption. Your PTA should have the following documentation for your local unit. These items should be maintained by the President, Secretary and Treasurer and available to board members as needed for donations and purchases.

- Legal Banking Letter (needed to open an bank account) or upon request of banker
- Florida SAles Tax Exemption Certificate - Form DR5
- Standard 501(c)(3) letter for donation requests;
- Stripe Account for credit card reader through Givebacks

Vendors or partners may request a W-9 form when working with your PTA/PTSA. You will use the information in your determination documents to complete the W-9.
Update BYLAWS (Every 3 years)
Every three (3) years, each PTA/PTSA must review their bylaws. You must use the current FL PTA Bylaws template. All FL PTAs have the same bylaws template, however, there are specific items within the template that are editable for your PTA as needed.
When it is time to review your bylaws, you will form a bylaws committee. They will review the bylaws and present their recommentation to a PTA/PTSA General Meeting.
The PTA/PTSA President, Secretary or designated officer will then submit the bylaws to FL PTA.

- If there are no edits, the committee reports that there were no edits and that the bylaws will be updated to FL PTA with no changes.
- If there are changes, the committee must report thos changes for vote at a General Meeting. Upon approval, the updated bylaws are changed.
The PTA/PTSA President, Secretary or designated officer will then submit the bylaws to FL PTA.

The Florida & National portion of dues are paid monthly when memberships are sold. As of July 1, 2025:
County dues are $70 per PTA/PTSA and due annually by October 31.
- Florida PTA dues are $1.25 per member
- National PTA dues are $3.25 per member
- Total = $4.50 per member
